Revolutionary surgical technique for perforations of the eardrum

ScienceDaily (Jan. 16, 2012) ? A revolutionary surgical technique for treating perforations of the tympanic membrane (eardrum) in children and adults has been developed at the Sainte-Justine University Hospital Centre, an affiliate of the Universit? de Montreal, by Dr. Issam Saliba. The new technique, which is as effective as traditional surgery and far less expensive, can be performed in 20 minutes at an outpatient clinic during a routine visit to an ENT specialist. The result is a therapeutic treatment that will be much easier for patients and parents, making surgery more readily available and substantially reducing clogged waiting lists.

"In the past five years, I've operated on 132 young patients in the outpatient clinic at the Sainte-Justine UHC using this technique, as well as on 286 adults at the University of Montreal Hospital Centre (CHUM) outpatient clinic," says Dr. Saliba. "Regardless of the size of the perforation, the results are as good as those obtained using traditional techniques, with the incomparable advantage that parents don't have to lose an entire working day, or 10 days or more off school in the case of children."

The technique, which Dr. Saliba has designated "HAFGM" (Hyaluronic Acid Fat Graft Myringoplasty), requires only basic materials: a scalpel, forceps, a probe, a small container of hyaluronic acid, a small amount of fat taken from behind the ear and a local anesthetic. The operation, which is performed through the ear canal, allows the body by itself to rebuild the entire tympanic membrane after about two months on average, allowing patients to recover their hearing completely and preventing recurring cases of ear infection (otitis). Because it requires no general anesthetic, operating theatre or hospitalization, the technique makes surgery much more readily available, particularly outside large hospital centres, and at considerably lower cost.

"With the traditional techniques, you have to be on the waiting list for up to a year and a half in order to be operated on. Myringoplasty (reconstruction of the eardrum) using the HAFGM technique reduces waiting times, cost of the procedure and time lost by parents and children. What's more, it will help clear the backlogs on waiting lists," Dr. Saliba says.

Perforations of the eardrum

Myringoplasty is surgical procedures to repair the tympanic membrane or eardrum when it has been perforated or punctured as the result of infection, trauma or dislodgement of a myringotomy tube (also known as a pressure equalization tube). Surgical repair of the perforation will allow the patient to recover his or her hearing and prevent repeated ear infections, particularly after swimming or shower. Traditionally, these procedures are performed using what are known as overlay and underlay techniques, which require hospitalization for at least one day, and 10 to 15 days off work. Every year in Quebec, some 750 myringoplasties are performed on adult or child patients.

Details of the study

This world premiere of a new form of eardrum surgery is based on results of a four-year prospective cohort study of 208 children and adolescents, 73 of whom were treated using the new HAFGM technique. This study was published on December 16, 2011 in the scientific journal Archives of Otolaryngology -- Head and Neck Surgery by Dr. Issam Saliba, otolaryngologist (ear, nose and throat or ENT specialist), surgeon and researcher at the Sainte-Justine University Hospital Centre affiliated with the Universit? de Montr?al, where he is also professor of otology and neuro-otology. Dr. Saliba is also a surgeon and researcher at the CHUM, where he conducted a similar study, applying the same HAFGM technique to cohorts of adult patients between 2007 and 2010, with publication in the August 20, 2008 issue of the scientific journal Clinical Otolaryngology and subsequently in the February 12, 2011 issue of The Laryngoscope. The University of Montreal and Sainte-Justine University Hospital Centre are known officially as Universit? de Montr?al and Centre hospitalier universitaire Sainte-Justine, respectively.

Younger Adults on Multiple Meds at Risk for Falls: Study (HealthDay)

TUESDAY, Jan. 17 (HealthDay News) -- Young and middle-aged adults who take two or more prescription drugs at any one time may be at increased risk for accidental falls, a new study finds.

The effect is similar to that seen in elderly people, according to the report published online Jan. 17 in the journal Injury Prevention.

In the study, the researchers looked at data from 344 working-age people who died or required hospital admission within 48 hours of an accidental fall at home in Auckland, New Zealand, between 2005 and 2006. They were compared with 352 randomly selected age-matched people in the general population.

The investigators found that people who took two or more prescription drugs were 2.5 times more likely to suffer an injury in an accidental fall than those who took one or no medicine.

The findings held true even after accounting for personal, social and lifestyle factors such as drinking, illicit drug use and amount of sleep, said Dr. Bridget Kool of the University of Auckland and colleagues.

The study also found that taking drugs to lower blood pressure and cholesterol appeared to triple the risk of a fall. Asthma inhalers, anti-inflammatory drugs, steroids and antidepressants did not increase the risk of a fall.

Whether the increased risk of falls associated with certain drugs were a result of the drugs themselves or the underlying conditions they were being used to treat is unclear, the researchers said.

Falls are a leading cause of injury and death worldwide, but most research focuses on older adults and children.

"The findings signal a need for greater awareness of the association between prescription medications and falls in younger adults, whether this is due to the medications, underlying conditions or a combination of both related factors," the study authors concluded in a journal news release.

(AP) ? Comedian Stephen Colbert says that just because his name won't appear on Saturday's ballot in South Carolina's Republican primary doesn't mean he couldn't become president, or even pope, someday.

In an oftentimes awkward, meandering interview on Sunday, Colbert told ABC's "This Week" that he established a presidential exploratory committee last week that consists of "someone who is good with explosives," a mountain climber and a brain in a jar. He likened himself to a "one-man Louis and Clark" expedition looking for his Sacagawea.

Colbert said if his quest for presidency doesn't work out, he would be willing to throw his weight behind one of the other candidates. He said: "All they have to do is come kiss my ring like they did Donald Trump's."

Colbert grew up in South Carolina.

Biologists a step nearer to solving the Parkinson's conundrum

ScienceDaily (Jan. 16, 2012) ? Scientists at the University of York have made a significant step forward in isolating the cause of Parkinson's disease in younger adults.

Research by a team in the University's Department of Biology found evidence that movement disorders, including tremor and slowness of movement (bradykinesia), associated with Parkinson's disease (PD) may be due to a defect in energy production in the nervous system. The advance may help to identify young adults who may be susceptible to the disease.

Parkinson's, the second most common form of neurodegenerative disease, principally affects people aged over 60, but some forms -- known as juvenile PD -- usually start in the 30-40 age group. One in 20 people diagnosed with Parkinson's are under 40 and such early onset PD is often inherited. Previous research has identified the genes which cause the disease and found them to be linked in a common pathway to failure of the mitochondria -- the power source within each cell.

In the latest research, part-funded by leading research charity Parkinson's UK and published in Human Molecular Genetics, scientists at York studied the effect that parkin, one of the genes which cause juvenile PD, has on the larva of the fruit fly, Drosophila.

They discovered that parkin faithfully models the locomotory defects of PD with a marked reduction in speed, and slower muscle contractions, reminiscent of bradykinesia.

Dr Chris Elliott, who led the study, said: "Our experimental evidence confirmed that this was due to a defect in the nervous system. This was important because previous work had suggested a big impact on the muscles, but PD is associated with neuronal failure."

The research team, which included undergraduate and postgraduate students, found that oxygen consumption and the production of ATP (the chief supply of energy inside cells) were drastically reduced. In response, lactate was increased.

The researchers also discovered that parkin larvae showed oxidative stress due to high levels of reactive oxygen species (ROS; also known as free radicals, such as hydrogen peroxide) which have been suggested as a key component of PD. The study suggested that relieving the ROS had only a marginal effect on mitigating slowed locomotion.

Dr Elliott added: "These findings show drastic failure in energy production by parkin larvae, and suggest that biochemicals related to lactate may be worth investigating as biomarkers for the progress of PD.

"We believe that the larval bradykinesia is a consequence of neuronal energy deficit, which leads to failure in neural communication. Oxidative stress is a consequence, rather than cause, of PD."

Dr Kieran Breen, Director of Research and Innovation at Parkinson's UK, comments: "This study shows just how vital models like the fruit fly are in helping us understand what happens to the nerve cells that are affected in Parkinson's.

"We already knew that mitochondria were important in Parkinson's but this research suggests that mitochondrial problems may be the root cause of the problems that lead to nerve cell death.

"So finding ways to protect and enhance the mitochondria may be the key to treatments that can slow or even stop Parkinson's in its tracks."

Peyton?s kid brother has climbed onto the upper echelon of quarterbacks with his play in the clutch, consistently converting third downs and generally making big plays.

For the Packers, the big plays were too few and far between.? And so the Giants, by winning the first playoff game in Wisconsin since the 2007 NFC title game, have a two-game postseason winning streak at Lambeau Field.

The Giants? win broke a 7-0 start to the postseason for the home teams.? It marks the second NFC upset of the weekend, with 36 and 37 points being the winning sum and neither the Saints nor the Packers ringing up those numbers.

Next up for the Giants, who lost to the Packers during the regular season by three, is a return to San Francisco, where the Giants fell by seven.? And if the Giants can turn the tables on the Niners, and if the Patriots can beat the Ravens, the rematch of Super Bowl XLII would also be a rematch of New York?s stirring November win, 24-20, at New England.

Of course, the Harbaugh brothers may have something to say about a Giants-Pats rematch.? Jim and John likely have dreams (or maybe nightmares) of a brother vs. brother return bout on the biggest stage in sports.

First class graduates from Oprah's South Africa school (Reuters)

HENLEY ON KLIP, South Africa (Reuters) ? Talk show host Oprah Winfrey proudly saw the first class of girls from disadvantaged backgrounds graduate on Saturday from her $40 million school in South Africa.

The Oprah Winfrey Leadership Academy for Girl celebrated the graduation of 72 girls. Wearing white dresses, the girls were cheered on by their families.

"The pride that I feel today is overpowering," Winfrey said at the graduation. "I have been on a mission my whole life to be able to give back what I have been given. Today I am fulfilling that mission."

"This class will prove that when you invest in the leadership of girls, you invest in a nation," said Winfrey, who herself emerged from childhood poverty to become one of the most influential women on American television.

The facility opened to much fanfare in 2007 with former South African President Nelson Mandela on hand along with U.S. celebrities including singer Tina Turner, filmmaker Spike Lee and comedian Chris Rock.

The graduation celebration, attended by Mandela's wife Graca Machel, was a rarity for the country's high schools.

"I'm very glad. I'm very happy. I feel blessed, you know. The girls look like angels," said a grandmother of one of the graduates.

Winfrey said, however, that if she had to do it over again she would not build a school from scratch, after a series of cost overruns drove up prices for the campus located about an hour's drive south of Johannesburg.

The school admits girls who show leadership qualities, have strong grades and come from poor families. Its facilities include a middle and upper school, state of the art laboratories, classrooms, a yoga studio and beauty salon.

"There are a lot of people, liberals, who lower their expectations and think that if you come from a disadvantaged background ... you have a disadvantaged brain. I know that is not true," the American entertainer told Reuters this week.

Winfrey's school was rocked when a matron was arrested about four years ago for suspected sexual abuse of students. She was later acquitted.

The school was among a handful in the country where all of the students passed a high-school graduation exam. All of its graduates will enter university, mostly in South Africa but with some going to the United States.

Despite a heavy investment in education since the end of apartheid in 1994, South Africa's overall system fares poorly, with thousands of schools still lacking basics such as books, desks, electricity and running water.

Most girls who enter the school system do not finish, and only about a quarter of all graduates do well enough on exams to qualify for entry to university.

"I was tested. We all were tested," Winfrey said, adding that now she is "a proud, proud momma."
